Immunization Schedule

Carousel Pediatrics recommends the following immunization schedule for all patients, whenever possible. Consult our medical staff if you have any questions.

3 months Check-up, Prevnar #1 (Pneumococcal)
4 months Check-up, DTaP #2, HIB #2, IPV #2, HepB #2
5 months Check-up, Prevnar #2
6 months Check-up, DTaP #3, HIB #3, IPV #3, HepB #3
8 months Check-up, PPD skin test (Tuberculosis), Prevnar #3
10 months Check-up, hemoglobin, vision photoscreen
12 months Check-up, urinalysis
15 months Check-up, Varivax (Chicken pox), MMR #1
(Measles, Mumps, Rubella)
18 months Check-up, DTaP #4, HIB #4, Prevnar #4
2 years Check-up
3 years Check-up, PPD skin test
4 years Check-up, DTaP #5, IPV #4, MMR #2, hearing and vision screens
10 years Check-up, TD (Tetanus)
5–18 years Check-up every year, PPD every other year
